Full-time OR Part-time, NO HOLIDAYS OR WEEKENDS PRIMARY PURPOSE The Pediatric Care Provider at Lorain County Health & Dentistry delivers primary medical care per the PCMH/NCQA model, to infants, children, adolescents, and young adults under the age of 19.
In addition to clinical assessment and treatment, the Pediatric Provider instructs and supports patients and their families to establish and maintain healthy habits for disease prevention and good health.
At Lorain County Health & Dentistry, medical providers typically develop long term relationships with patients and their families/caregivers.
They make individualized treatment plans, engage patients/families in the planning process, and offer ongoing support.
The Pediatric Provider makes specialty referrals when necessary, and provides care in accordance with Lorain County Health & Dentistry's philosophy of partnership with patients while integrating its values of Respect, Quality, Compassion and Hope.
EDUCATION AND CERTIFICATION REQUIREMENTS Doctor of Medicine (MD) or Osteopathy (DO) is required; or Masters of Science in Nursing with Advanced Practice Certification required.
Must be board certified in pediatrics.
Must be licensed in the State of Ohio and meet all requirements for credentialing and privileging.
Must maintain appropriate continuing education requirements as established by State Board of Medical Examiners.
Must have BLS certification.
SKILL AND EXPERIENCE REQUIREMENTS A minimum of three years of experience in a primary care setting, preferably a non-profit community healthcare environment.
Must have computer skills and the ability to become proficient in software programs specific to Lorain County Health & Dentistry, i.
e.
, NextGen EHR.
Must have the skill and credibility to serve as a patient advocate and be a role model for change.
Must maintain a calm and professional demeanor.
Must communicate enthusiastically with patients, their families/caregivers, and support staff.
Must be responsive and appropriate in working with a culturally-diverse patient population and staff.
Must have excellent communication skills.
Must be able to independently coordinate multiple tasks.
Must be able to work cooperatively with others.
Must have valid driver's license and satisfactory driving record.
Bilingual, Spanish-speaking is a plus.
